Ordering forms for professionals
IMPORTANT INFORMATION
We are unable to accept orders from countries where no local distributor is available. If you are ordering from outside Finland, the order must be submitted to your local distributor.
- PLymed’s custom-made products may be ordered by our measurement-trained distributors as well as other healthcare professionals with adequate measurement expertise.
- pWe can only receive information related to manufacturing and patient identification in written form to ensure reliability.
- pOrders are registered upon receipt and transferred to production as soon as all required information has been provided on the order forms.
- pBy placing an order, you consent to the transfer of personal data to Lymed for the purpose of manufacturing the products. For more information see our Privacy Policy
If you are ordering from within Finland, the completed order form can be sent: by email to info@lymed.fi or by post to: Pyhäjärvenkatu 5, 33200 Tampere, Finland.

Our order forms can be used to order custom-made medical devices and CE-marked standard-size medical devices.
For custom-made devices, the measurement and specification data form the written prescription (specification) that serves as the basis for manufacturing in accordance with EU Regulation (EU) 2017/745 (MDR). For standard-size devices, the information provided constitutes standard order information.
Ordering a custom-made product requires product knowledge and measurement expertise.
